A SDA is an assessment conducted by a Ministry of Health (MOH)-accredited assessor to determine if an individual is severely disabled. It evaluates the ability to perform at least 3 out of 6 Activities of Daily Living (ADLs) - feeding, dressing, showering, toileting, moving around, and transferring. SDA is required when applying for long-term care severe disability financial assistance schemes like CareShield Life and ElderShield, and filing a claim for supplement schemes of CareShield Life and ElderShield administered by private insurers.
The Complimentary Benefit is not transferable. While the membership account belongs to you, you can use your care points to pay for products or services booked for your family members.
Yes, you can utilize the membership benefits for your family member who may need the service(s). This includes the complimentary SDA and FAR in the event if it is your elderly parent who is the one suffering from a disability.
A FAR is a medical document used to evaluate a person's ability to perform Activities of Daily Living (ADLs) - such as bathing, dressing, and feeding. It acts as a mandatory assessment tool for accessing government financial support for long-term care, specifically for people with mild to moderate disabilities. FAR is needed when applying for schemes such as the Home Caregiving Grant, Pioneer Generation Disability Assistance Scheme (PioneerDAS), Migrant Domestic Worker Levy Concession and Enhancement for Active Seniors (EASE) program for installing home modifications.
